The purpose of this form is to allow the agents, agencies or sponsor(s) listed below to make inquiries on behalf of the student during or after the application process. Ensure this form is completed and signed by the student no later than 15 days after the application has been submitted on the University's website and that the student information matches the University application exactly. Do not submit a Direct Entry Waiver form for any major available via INTO Apply.
